The Church of England.
Man is the state or its bishop
Started with a bad marriage, this religion came out of Catholicism, and still retains much of its roots. It is the official state religion of England, and its head is the Bishop of Canterbury.
Reformation moved England from Roman Catholicism to Anglicanism, which pretty much made the monarchy the head of the church.
It has a liturgical form of worship, with set schedules and a Christian calendar, and its members all share in worship via the Book of Common Prayer.
It utilizes several sacraments, such as Baptism and Holy Communion. In all liturgical churches, there is a danger of allowing the form of religious ceremony to replace the personal application of faith.
In the United States, this religion is known as the Episcopal Church.
